Application Forms

An application form is available in PDF format for admission to each grade of membership, or to upgrade to a new category. Former members (ACSI and MCSI) seeking to rejoin the CISI should use the Membership Reinstatement Form to reactivate their membership. Fellows (FCSI) looking to reinstate will need to reinstate as Members (MCSI). Please click here to access Annex 3 which forms part of the membership declaration.
